RMC Home Health Care provides patients with a professional continuum of treatment based on their individual needs in the comfort of their own home.
With more than a century of combined home health care experience, RMC Home Health Care offers patients personalized services to meet their individual needs. In addition to basic functions ranging from checking vital signs to diet planning, services include:

The goal of RMC Home Health Care is to work with the patient’s health care providers to make sure that patients receive a coordinated approach to treatment in their home. Each patient has a treatment team that includes the patient’s physician, as well as a medical social worker who coordinates community social services specifically for that patient. In addition, the patient’s nurse gets to know the patient and his or her condition so well that any changes or special situations that may arise are taken care of promptly. Such a continuity of care only ensures a much greater chance for enhanced patient outcomes and recovery.
Who is Eligible to Receive RMC Home Health Care?
To receive RMC Home Health Care services:
- A patient must be homebound
- A patient must require skilled care
- A patient must have physician orders for home health care
RMC’s specially trained nurses are available 24-hours-a-day, seven-days-a-week to deliver services. Typical home health care patients include patients who are:
- Recovering
- Disabled
- Chronically Ill
- Terminally Ill
Services also are provided for individuals in need of medical, nursing, social or therapeutic treatment and/or assistance with the essential activities of daily living.
Does Insurance cover RMC Home Health Care?
Payer sources for RMC Home Health Care include Medicare, which pays 100 percent of home health care for eligible patients, Medicaid, Veteran’s Administration, private insurance and private pay.
RMC’s Home Health Care is accredited by Medicare and the Joint Commission on Accredited Health Care Facilities, which measure the highest standards in the medical industry.
